The Gerald L. Ignace Indian Health Center (GLIIHC) is located just south of downtown Milwaukee, Wisconsin, which has a population of 595,351 (2017, est.). GLIIHC is located in the urban community where most American Indian/Alaska Natives (AI/AN) who live in the area reside. The community surrounding GLIIHC is a mixture of lower- and middle-income families. Although the local AI/AN population is reported to be 18,239 people (Urban Indian Health Institute Report, 2017), or about 1/3 of the state’s AI/AN population. The general patient population at GLIIHC is lower income, with 40% making at or below 200% of the federal poverty level. The Medicaid (Title XIX) enrollment rate is 42%; 11% of patients carry no insurance at all. Related to health promotion and disease prevention (HP/DP), the GLIIHC has noted a steep drop-off in screenings (colon cancer, cervical cancer, breast cancer, others) since the start of the COVID-19 pandemic. Patients with chronic conditions such as hypertension, diabetes, asthma, and others have also struggled to manage and maintain their health during COVID-19. This has led to stepped-up efforts to connect with patients and prospective patients to get them back into care in the most COVID-safe ways possible. Patients are behind on recommended vaccinations across the age span, as well, due to COVID-19. Getting children caught up on recommended vaccines is a high priority, as is reaching elders eligible for pneumonia and flu vaccinations. The HP/DP programs at GLIIHC are key pieces to getting—and keeping—patients in the best health possible from a prevention perspective. The Indian Health Service 4-in-1 grant funding has allowed GLIIHC to greatly expand medical and behavioral health services and also provide a variety of culturally responsive programming. These offerings include culturally based substance use disorder services i.e. Wellbriety Groups/Native American 12-Step. In our pre-covid-19 service environment we also provided regular offerings of: Women’s Talking Circle, Evening Talking Circle, Beading Circle, Fan Making Classes, Hand-drum Making Classes, 1:1 patient Smudging/Purification, Sweat Lodge Ceremonies (for coed, and women only groups), 1:1 Spiritual Counseling; Native Wellness Garden traditional food planting and harvesting, and our Work Out Low Fat Elders (WOLFE) Wednesdays. Program goals will include: • HP/DP Goal 1: To increase screenings and other preventive health measures for disease prevention. • HP/DP Goal 2: To promote healthy eating habits in community members and partner organizations. • Immunization Goal: Increase rates of age-appropriate immunizations in children, adults. • Mental Health Goal: Increase access to and uptake of evidence-based mental health services in the AI/AN community of greater Milwaukee. • Substance Abuse Prevention Goal: To increase access to evidence-based and culturally relevant substance abuse services for the Urban Native American population of the greater Milwaukee area.
