SDPI Community-Directed Program - We are requesting the that the Makah Tribe/Sophie Trettevick Indian Health Center be awarded continuing funding in the amount of $141,580.00 from the 2023 Special Diabetes Program for Indians. This funding is necessary in order to continue providing excellent care to diabetic patients, as well as prevention activities in the community. The following are important aspects of our program dependent on this continuing funding. The Sophie Trettevick Indian Health Center (STIHC), owned and operated by the Makah Tribe, is located in the seaside fishing village of Neah Bay on the Makah Indian Reservation. One narrow, serpentine road winds along the coastline to connect the village to the Olympic Peninsula. The user population for STIHC is 2,483 patients, which includes non-tribal members, college students, and tribal members who utilize the STIHC medical services but do not actually reside on the reservation. Our Diabetes Roster currently has 79 patients receiving care for diabetes at STIHC. We provide case management, education, and routine medical care for these patients. Additionally, our Diabetes Nurse and Community Health Program provide Home Visits for Diabetic patients who need a more intensive care plan as ordered by the medical provider. We are able to provide much of the routine medical exams and care locally at STIHC. We contract with an out of town Ophthalmologist to provide Dilated Diabetic Eye Exams. However, Cardiology and other specialty care, as well as appointments with certified diabetes educators must be referred to an out of area provider. The closest providers for these types of referrals are located in Port Angeles, which is approximately 2 hours from Neah Bay. Our Diabetes Program is tasked with assisting these patients with scheduling and transport to these appointments.
