Special Diabetes Program for Indians - Pueblo of Zia Project Abstract Summary The Pueblo of Zia is a federally recognized Indian Tribe located in central New Mexico. Zia Pueblo sits atop a mesa overlooking the Jemez River, 17 miles northwest of Bernalillo on Highway 550 and 35 miles northwest of Albuquerque. Zia Pueblo is one of 19 Pueblos in the State of New Mexico, one of seven Keres speaking Pueblos, also known as the “Home of the Zia Sun Symbol.” Zia Pueblo spans more than 121,080 acres of Indian land, 675 tribal members currently live on the reservation. Zia Pueblo members are strong, active participants in their traditional cultural/customs, well-known artisans for traditional pottery making. Pueblo men grow their own vegetables such as chili, corn, squash, watermelons, etc. Our purpose is to seek resources needed to provide community-based nutrition education, diabetes related education/services, physical activity education and transportation services to address diabetes related problems, obesity, and other health issues. SDPI funds will be utilized to provide these services/education to the Zia community members, age 7-85 yrs. old. Based on the needs assessment from community members and Leadership/Administration’s participation, our selected best practice is Nutrition Education. The target population for the best practice is Zia community members, age 7-85, who have diabetes and/or at risk of developing diabetes, live on/off reservation and are provided services/education at community events, in the clinic or in the home. Medium to High Intensity. Collaborate with Global Nutrition LLC to provide Nutrition Phone Consultations & One-on-One/Group Nutrition Education, Zoom Healthy Snack making in collaboration with Zia Health Task Force & Five Sandoval Prevention Program, Traditional/Western Cooking Classes in collaboration with the Zia Language Program and Health Fairs will be in collaboration with Zia Health Clinic & other health providers. Our measure will be the number and percent of individuals in our target group who receive nutrition education. Other Services will be provided for Diabetes Related Education through the Mail Out Process with educational materials and self-care packages for Foot, Eye, Dental, Medication, Physical Activity, & Stress Management. We hope for better diabetes management, improved A1C numbers and movement to lessen sedentary conditions. Physical Activity Education/Services will be provided through the fitness center with personal training and group exercise such as challenges, hiking, run/walks, family fitness nights, etc. To decrease health risk factors in the community. Transportation Services for community members to get to their medical appointments and continue good health care services for our people. We will use collaboration methods to assure efficient use of resources and existing partnerships.
