The New Mexico Family Planning Program (NM FPP) has been a Title X grantee since 1981 and has a strong infrastructure to continue providing quality family planning services to New Mexicans. This is an application for a 5-year project through Funding Opportunity Number: PA-FPH-22-001, CFDA Number 93.217. While NM FPP is applying for a grant award of $9,392,212.00 per year, including $3 million for an electronic health record system upgrade for full FPAR 2.0 compliance and assistance with advancement of Title X clients’ access to additional non-Title X services and promotion of health equity, if the additional $3 million is not acceptable, NM FPP wishes to be considered for base Title X funding of $6,338,172. The Project targets underserved and hard-to-reach populations such as teens, males, and low-income individuals. NM FPP projects that services will be provided for 10,000 unduplicated clients in 2022. Title X services are provided in 41 Public Health Offices, with two outreach sites, and 15 contract clinic sites including primary care clinics, and school-based health centers. Community-based education is provided at various sites, from schools to government entities to community organizations. The long-term program impacts include the following: 1. To reduce the teen birth rate among girls ages 15 -19 by 3% annually. By the year 2025, reduce the teen birth rate among female teens ages 15-19 to no more than 17.8 per 1,000. 2. Reduce the unintended birth rate among women ages 13-44 by 0.5% annually. By the year 2024, reduce the unintended birth rate among women aged 13-44 to no more than 44.3%. 3. Annually, a minimum of 65% of sexually active female family planning clients under age 25 years will be tested for chlamydia.
