Unity Health Care's District-wide Title X Family Planning Services Program - As the largest network of community health centers in Washington, D.C. (District), Unity Health Care, Inc. (Unity) provides a full-range of health and human services to meet the needs of communities through a network of over 22 health care service sites including community health centers, homeless service sites and school-based health centers. Our team of diverse and compassionate health professionals put Unity’s values into action every day to bring whole-person care and wellness to nearly 100,000 patients through nearly 400,000 visits annually. Deeply rooted in the District’s neighborhoods for over 35 years, Unity strives to promote healthier communities through compassion, comprehensive primary and specialty health care and wrap-around services, regardless of ability to pay. Unity is a Federally Qualified Health Center (FQHC) and has successfully managed Community Health Center and Healthcare for the Homeless Federal grant programs for over 25 years. Our mission is also demonstrated through the successful implementation and expansion of our Title X Family Planning Services Program during the past 15 years. Our patient population is the low-income, medically vulnerable residents of the District in need of family planning and related preventive health services. Unity operates 22 health care sites and a mobile unit, which provide family planning services in all eight wards of the District. Our Title X subrecipient partners and community-based partnerships further our reach throughout the District. Our health centers are recognized as patient-centered medical homes and we offer comprehensive client-centered information, counseling and support for achieving and avoiding pregnancy. To assure maximum accessibility, our sites are strategically located in areas populated by large numbers of people living in poverty and/or experiencing disproportionately high rates of adverse health risks. Our institutional approach focuses on ensuring that our programs are accessible, of high quality, culturally appropriate, and responsive to the needs of our patients. To continue building on the momentum generated by our family planning program, we request continued grantee status in response to the Department of Health and Human Services Office of the Assistant Secretary for Health (HHS/OASH) Title X Family Planning Services Grants. Unity requests $1,944,065 to support coordinated efforts to meet the family planning and related health needs of 51,239 District men, women and adolescents. As the District’s Title X Grantee, Unity will continue to utilize our clinical expertise, administrative efficiency and dedication to our mission to effectively engage community partners and provide support for Title X and other community agencies so we may collectively meet the reproductive and sexual health needs of the District’s men, women and families. During the next program period, Unity will continue to advance health equity by providing accessible, high-quality family planning services that comply with Title X Statutes, regulations and legislative mandates, and plan to use encounter-level family planning data to identify areas for improvement and conduct continuous quality improvement activities related to family planning service delivery and patient satisfaction. We are committed to reaching low-income and underserved communities in need of comprehensive family planning services. Through coordinated community outreach and education, quality clinical services, and committed partnerships, we aim to meet OPA’s program priorities for family planning clients.
