Title X Family Planning Services - For more than 45 years, sexual and reproductive health providers across Massachusetts (MA) have received Title X funds to deliver high-quality, evidence-based care to marginalized populations. A state agency with proven administrative, management, and clinical capability to administer the proposed Title X Family Planning Program, the MA Department of Public Health (MDPH) proposes a statewide service area with 81 clinics operated by 16 organizations located in at least 36 high-priority communities in all seven regions of the state. MDPH will serve as the facilitating organization and administrator of Title X funds, contracting directly with subrecipients to provide clinical services to X clients annually. There are 379,421 clients across MA who could benefit from Title X family planning services, including adolescents aged 15-17 and individuals aged 18-44 below 100% of the federal poverty line who live in high need communities identified by MDPH’s sexual and reproductive health (SRH) Need Index. MDPH Sexual and Reproductive Health Program’s (SRHP) overall vision is that low-income MA residents prevent unintended pregnancies and plan and raise families in safe, healthy communities by accessing a broad range of medically approved clinical care and community resources that are Title X compliant, client-centered, voluntary, confidential, accessible, equitable, and that support bodily autonomy. MDPH’s goals include (1) decreasing sexual and reproductive health inequities in MA, including in unintended pregnancy, STI incidence, maternal morbidity and mortality, and infant mortality through the delivery of high-quality Title X services; (2) providing comprehensive oversight activities including QI/QA activities and program monitoring, data collection and reporting, and training and technical assistance to assure that all subrecipient agencies are informed, prepared, and implementing Title X services in accordance with Title X statute, regulation, legislative mandates, and program priorities, MDPH SRHP standards, and national, evidence-based standards of care; and (3) increasing the sustainability of the Massachusetts sexual and reproductive health delivery network. MDPH selected program subrecipients based on their capacity to serve those communities with the highest need. Subrecipients have demonstrated experience and commitment serving three priority populations (low-income, adolescents, and people of color), as well as LGBTQ+ people, people who use drugs, homeless individuals, survivors of sexual assault/trafficking, currently/formerly incarcerated youth and young adults, those in rural areas, unaccompanied minors, and populations with intersecting needs. MDPH provides training, technical assistance, data collection, and evaluation to subrecipients as they provide trauma-informed, gender-inclusive care, referrals for social determinants of health and specialty medical needs, health navigation, insurance enrollment, outreach, and case management. Services include contraceptive services, pregnancy testing and counseling, sexually transmitted infection and HIV services, assistance with achieving pregnancy, preconception services, basic infertility services, related preventive health services, and adolescent-friendly health services. Family planning methods include natural family planning, short-acting hormonal, long-acting reversible contraception, emergency contraception, and barrier methods. All Title X statutes, regulations, and legislative mandates are embedded in the MDPH SRHP Standards and reinforced through subrecipient contract conditions, onboarding, and training activities.
