The Neighborhood Improvement Project, Inc. DBA Medical Associates Plus (MAP), established in 1997, is a not-for-profit Federally-Qualified Community Health Center (FQHC) headquartered in Augusta, Richmond County, Georgia. MAP is a comprehensive healthcare facility and provides primary health care; oral health care; behavioral health and substance abuse services; infectious disease; pulmonology; endocrinology, infusion treatment; cardiology; endocrinology; and diabetes/health education specialty care; pediatric care; women?s health; and 340B discounted pharmacy services. After nearly five years as a data reporting partner to a statewide Title X project, and a new grantee as of 2018, MAP seeks to increase its family planning capacity to best serve the needs of women of childbearing age and men of all ages. Title X funds will support expansion of staff, including health education, leading to an annual increase of a minimum 15% in patient encounters by project end.In 2017, 3,195 women received family planning support at MAP, with 4,552 visits for contraceptive management (Uniform Data System Report 2017). This total is more than four times the 2016 total of 788 patients for 1,030 visits for contraceptive management (UDS 2016). The increased demand for services, coupled with high rates of sexually transmitted infections, teen pregnancy, high maternal mortality and high prevalence of low-weight births in the community, all indicate a need for improved access to quality care and culturally-competent services. MAP proposes to serve 3,400 patients, equating to 5,100 visits for 2019-2020.
