Fall Education and Prevention Program for the Stockbridge Munsee Community Health Department - The Stockbridge Munsee Community Health Department seeks funding to continue its Fall Prevention Program, an evidence-based initiative designed to reduce fall risk among community elders. The program will offer Bingocize and other evidence-based interventions at the Elderly Center and Fitness Center to improve strength, balance, and mobility. Participants will receive incentives to encourage consistent attendance and engagement. Participant progress will be monitored using pre- and post-program fall risk assessments based on the CDC’s STEADI (Stopping Elderly Accidents, Deaths, and Injuries) initiative. In addition to structured exercise programs, the project will conduct home safety evaluations for community members, focusing on identifying and removing environmental hazards. Fall prevention equipment, such as grab bars, non-slip adhesives, night lights, and stair treads, will be installed to improve household safety. The program will also provide in-person education sessions led by guest speakers and distribute educational materials to raise community awareness about fall prevention. Project staff will collaborate with healthcare providers to strengthen fall risk screening practices, promote the use of STEADI assessment tools, and increase referrals for home safety assessments. This integrated approach aims to reduce fall-related injuries, increase elder participation in prevention programs, and build long-term capacity for evidence-based fall prevention within the Tribal Health Department.
