(PHS) celebrated its 53rd anniversary as a Federally-qualified health center in March 2023. Serving a widely diverse seven-county service area, its reach extends throughout Alamance, Caswell, Chatham, Lee, Orange, Person and Randolph counties. Piedmont Health Services, Inc.’s Commission-accredited, PCMH Level 3-recongized health centers are located in communities facing multiple barriers to healthcare access. More than three-quarters of its patients are racial and ethnic minority populations that face significant health care disparities; more than half are uninsured; nearly half have preferences to receive care in a language other than English, and nearly all who choose to apply for sliding fee care reveal income at or below 200% of the Federal poverty level. PHS’ direct scope of services includes family medical, dental, and integrated behavioral healthcare, an on-site 340b pharmacy, lab, and enabling services including WIC/nutrition, care management, interpretation support, eligibility assistance, migrant/seasonal farmworker (MSFW) outreach, and the SeniorCare Program of All-inclusive Care for the Elderly (PACE). PHS proposes an equipment-only project which will expand its ability to serve its community, especially during weather-related emergencies when fixed sites are compromised or during other declared emergencies. Moreover, the project will purchase and equip a 2-medical exam room, wheel-chair accessible mobile medical unit for operations with a focus on PHS' high-demand southern service area. Total project costs include procurement of the medical van and additional clinical and non-clinical equipment items to ready the unit for service delivery. While the project period for the award is 3 years, PHS feels based on its experience with a prior mobile unit project, that it can complete the project in a 19-month time frame.
