Enhancing State No Wrong Door Systems for Efficient Access to Long-Term Services and Supports - The North Carolina Center for Health and Wellness (NCCHW), in collaboration with the NC Division of Aging, Community Care Hub, and regional Councils of Government, will modernize and expand North Carolina’s No Wrong Door (NWD) system to improve equitable access to long-term services and supports (LTSS). This initiative will enhance service coordination, reduce administrative burdens, and strengthen person-centered care through technology-driven solutions and cross-sector partnerships. Over two years, the project will improve IT infrastructure by leveraging NCCARE360 and exploring interoperability with other systems. These enhancements will streamline referrals, improve data sharing, and optimize intake, assessment, and eligibility processes. By modernizing the NWD system, the initiative will enhance efficiency, reduce duplication, and maximize resources. Key objectives include: (1) Aligning NWD goals with state initiatives, including the NC Multi-Sector Plan for Aging; (2) Expanding sustainable funding, including Medicaid Administrative Claiming; (3) Strengthening partnerships to increase service capacity; (4) Enhancing IT capabilities to support a broader range of LTSS services; (5) Optimizing NCCARE360 and exploring additional systems for efficiency; and (6) Assessing IT systems for real-time data exchange and administrative burden reduction. Expected outcomes include a more efficient referral system, increased provider and community-based organization (CBO) participation, improved service access, and a sustainability strategy. Products include a referral process improvement plan, IT enhancements, provider training, and marketing materials. This initiative will strengthen North Carolina’s NWD system, ensuring individuals and caregivers can access the right services at the right time while reducing barriers to LTSS across the state.
