Wisconsin Lifespan Respite System Improvement Framework Project - Wisconsin will strengthen its Lifespan Respite system so family caregivers can more easily find and use timely, person-centered respite supports. The project addresses uneven service availability, caregiver stress, workforce shortages, fragmented referrals, and barriers related to cost, transportation, disability access, technology, and awareness. Using the Wisconsin Lifespan Respite Continuous Improvement Framework (WLRCIF) in Attachment 1, the project will use mapped data, community discovery, targeted strategies, workforce development, evaluation, and sustainability planning to build on prior Lifespan Respite investments, DHS and RCAW leadership, ADRC/No Wrong Door infrastructure, Tribal and community partnerships, respite grants, Registry expansion, GIS mapping, and continuous quality improvement. Expected outcomes include improved caregiver well-being, increased access, expanded provider capacity, stronger referrals, better data use, and a more sustainable statewide respite system. Products will include county and regional profiles, GIS dashboards, outreach and training materials, implementation tools, evaluation findings, an implementation guide, replication toolkit, and sustainability plan. The anticipated three-year period of performance is September 30, 2026, through September 29, 2029.
