Parenting Case Manager TBN. Salary requested is for 12 months (the second project period) at 50% effort. The Case Manager will work at FC and report to Dr. Karol Kaltenbach, the Executive Director who will coordinate care with the case manager. The Case Manager will also work closely within the current case management team at FC, but specifically will facilitate parenting support services. The Case Manager will also work closely with the Project Director and the Pediatric Social Worker at the pediatric practice to insure that Mothers, who are referred by the pediatric practice to child early intervention, individual mental health counseling for mother or child, family counseling, parenting support services, etc. are facilitated to attend the referral and will provide follow up information back to the pediatric practice social worker to inform the practice staff and physicians of patient follow up information.
