West Virginia Medicaid and WVCHIP Reentry Program - The West Virginia (WV, State) Bureau for Medical Services (BMS), a division under WV’s Department of Human Services (DoHS) is applying for funding under Section 206 of the Consolidated Appropriations Act (CAA) of 2024 to help the State strengthen the infrastructure needed to support continuity of care for Medicaid and WV Children’s Health Insurance Program (WVCHIP) beneficiaries following incarceration. BMS will lead WV’s Continuity of Care Project as the State’s Medicaid agency, working in collaboration with the Division of Corrections and Rehabilitation (DCR), its Bureau for Juvenile Services (BJS), and the West Virginia Health Information Network (WVHIN) to implement grant requirements and activities. WVHIN will be a subrecipient for this project. This funding will support the implementation of Section 5121 of the CAA of 2023, align 1115 demonstration systems, and facilitate access to Medicaid/WVCHIP services for incarcerated youth and adults in WV. The primary goal of this project is to help the State ensure eligible individuals receive timely and coordinated services upon reentry by developing policy infrastructure, enhancing operational systems for Medicaid and WVCHIP enrollment, and improving data-sharing and tracking mechanisms. To achieve this, WV plans to: · Establish policies, procedures, and standards to support the timely delivery of 5121 services across correctional settings · Develop and, where possible, automate systems for enrollment, service coordination, care management to reduce administrative burden · Build monitoring and oversight capacity to track and evaluate 5121 service implementation The total budget for this project is $4,533,900. Funds will be used to support interagency collaboration, implement sustainable operational strategies, enhance bidirectional data exchange between corrections and providers, and develop a thorough monitoring and quality assurance framework. In Year 1, BMS will prioritize collaboration with partners, finalize policies, and establish systems for 5121 service delivery. Over the four-year grant period, BMS will enhance systems and data-sharing capabilities, build corrections infrastructure, and strengthen Medicaid/WVCHIP enrollment. These strategies will improve interagency collaboration, data-sharing, and communication, ensuring eligible juveniles access critical services upon reentry. This will ultimately improve coordination, oversight, and service continuity, aligning with state Medicaid quality frameworks.
