The South Dakota Department of Social Services, Division of Medical Services, (South Dakota Medicaid) is the single state Medicaid agency (SMA) and will serve as the lead organization for this opportunity. South Dakota Medicaid is one of the state’s largest healthcare purchasers, providing health care coverage for over 140,000 of South Dakota’s approximately 884,000 residents.
South Dakota Medicaid seeks to develop a coordinated, data-driven approach to ensure eligible juveniles experience seamless transitions to essential healthcare services post-incarceration, reducing health disparities and promoting sustained, positive health outcomes.
South Dakota Medicaid will collaborate with the South Dakota Department of Corrections, tribal juvenile detention facilities, and county-level juvenile service centers to strengthen data sharing and address identified gaps in healthcare access.
Fund Use Description:
South Dakota has conducted stakeholder outreach and worked towards putting the necessary processes and coverages in place to implement the 5121 requirements for January 1, 2025. The initial project phase will further build on this work. The state will conduct a systemic analysis informed by stakeholder engagement to identify barriers and challenges that still may remain. The results of the systematic analysis will be compiled into a report with recommendations and an action plan for addressing specific barriers and challenges to meeting the 5121 requirements.
Phase Two will focus on implementing the report's recommendations. Recommendations may pertain to items such as enhancing access to care for eligible juveniles, data-sharing, or systems enhancements. Action items to address identified barriers could include enhancements to processes, purchase and utilization of telemedicine systems and equipment, development of additional educational and training materials for carceral systems, and enhancements to data sharing processes or systems.
This approach will enable South Dakota Medicaid and its partners to ensure that juveniles in detention experience seamless healthcare transitions upon release, ultimately reducing recidivism and improving long-term health outcomes.

South Dakota Medicaid is asking for a total budget of $1.5 Million.
