The South Carolina Department of Health and Human Services (SCDHHS) is the single state agency that administers the Healthy Connections Medicaid program. This makes SCDHHS the state's largest health care payor; currently covering 60% of births in the state, and uniquely situated to implement lasting transformation in the maternal health space. Additionally, SCDHHS leads the Birth Outcomes Initiative (BOI), which serves as South Carolina’s perinatal quality collaborative, and is supported in this effort by over 100 stakeholders throughout the state.
SCDHHS applied for this Cooperative Agreement funding opportunity to advance maternal and infant health outcomes in South Carolina. SCDHHS is requesting $17 million dollars to leverage the work happening across our state and align around the TMaH model to build a sustainable, best in class, service array for mothers and babies in South Carolina. SCDHHS will work through the state's Managed Care Organizations to build upon a successful foundation for value-based arrangements and quality improvement through Learning Collaboratives, and apply this funding to invaluable staff infrastructure, participation incentives, anchor partnerships, innovation partnerships, and AIM implementation.
