Project Abstract Summary Egyptian Health Department (Lead Organization) Illinois Department of Healthcare and Family Services (HFS) (State Medicaid Agency) InCK Model The Egyptian Health Department (EHD), in partnership with the Illinois Department of Healthcare and Family Services (HFS), Illinois’ single state Medicaid agency, proposes to implement the Integrated Care for Kids (InCK) Model in the southeastern counties of Saline, Gallatin, White, Hamilton and Wayne. The proposed InCK Model proposes to address the total needs of the Medicaid population 0-21 years of age by providing integrated care coordination to improve child health, reduce substance use disorders and opioid use, and reduce out of home placements. EHD will utilize the requested funds $16 million over 7 years to establish an Integrated Services Hub (i-Hub) to link to children and families with local and statewide resources; partner with the Illinois Chapter of American Academy of Pediatrics to create a local training collaborative for primary care providers on completing behavioral health screening instruments as well as referring for services after screens; provide Wellness Coaches to provide support to the chronic and medically complex children; utilize a comprehensive assessment tool for Integrated Assessment and Treatment Planning and stratification; utilize Peer Support Providers across all Service Integration Levels; utilize Care Mapping processes; and provide integrated care coordination and in-home based services. EHD will partner with HFS for planning for sustainable Alternative Payment Models (APM) that will ensure better health outcomes as well as cost savings.
