Rural hospitals play a critical role in providing communities with access to care. Unfortunately, the current financial state of many rural hospitals does not allow for the improvement of care delivery. This funding opportunity will allow Pennsylvania Department of Health to ensure that rural Pennsylvanians will have access to high-quality care and local vitality by keeping rural hospitals open. The Pennsylvania Rural Health Model was developed to test the effect of delivery transformation of rural hospitals and population-based payments (in the form of all-payer, prospective global budgets) on quality and cost of care for rural Pennsylvanians. Under this model, the Commonwealth is committed to achieve population health outcomes, access quality measures and targets, model financial, rural hospital and payer participation scale targets. The initial stage of this Model was to support the pre-implementation phase. Currently, the Model is finalizing its pre-implementation phase with five participant rural hospitals in Performance Year (PY) 1 and twelve additional hospitals undergoing enrollment for the upcoming PY 2.
