PROJECT OBJECTIVE: The Center for Human Services (CHS) is seeking funding in the amount of $199,930.00 to support PATHWAYS; a comprehensive strength based transitional living pr9gram for youth focusing on shelter and support services for youth 18-21. Using a youth development approach, PATHWAYS seeks to strengthen individual assets and foster independency through employment and skill development. APPROACH: PATHWAYS staff incorporate participant feedback to establish the most effective approach to reach the target population. Services include: safe shelter through transitional living in supervised apartments, case management, counseling, employment development, advocacy/referrals and life skills training. RESULTS/BENEFITS EXPECTED: PATHWAYS is an essential component of local efforts to reduce youth homelessness, foster independency, and keep youth in school or job training programs. Specific anticipated expected results include: o 12 to 20 youth will receive comprehensive residential services including safe shelter in supervised apartments, food, clothing and case management in a 12 bed setting. o 12 to 28 additional youth will receive non-residential services including counseling, job development assistance, life skills instruction, referral information and other support services. o A minimum of 10 youth served will graduate from the program per year to independent living settings or a minimum of 25% served. 12 to 30 youth will be enrolled in an academic or trade school program. 15 to 40 youth will be assisted to obtain and keep employment. 20 youth will participate in volunteer opportunities in the community.
