Objectives: This funding was appropriated under the American Recovery and Reinvestment Act of 2009 (Public Health Service Act, 42 U.S. Code 241(a) and 247b (k) 2) to reduce Chronic Disease risk factors, prevent and delay chronic disease, promote wellness, and better manage chronic conditions. This initiative will address the following: * Increased levels of physical activity * Improved nutrition (e.g. increased fruit/vegetable consumption, reduced salt and transfats); * Decreased smoking prevalence and decreased teen smoking initiation; and * Decreased exposure to secondhand smoke. Communities funded under this announcement will accomplish this by implementing population-based approaches such as policy, systems, and environmental changes across 5 evidence-based strategies – access, media, point of decision information, social support services, and price – in both schools and communities. Both components are necessary to achieve behavior change in youth that sustain from school settings into family and community settings, and supports sustaining healthy behavior into adulthood. Communities funded by this announcement will receive a menu of evidence-based interventions along with support and tools from the Centers for Disease Control and Prevention (CDC) to strengthen and develop effective strategies tailored to their needs. The cooperative agreements will address the “Healthy People 2010‿ focus areas of obesity, cardiovascular disease, tobacco, and educational and community-based programs; the Health Protection Goals of “Healthy People in Every Stage of Life‿ which states that “all people, and especially those at greater risk of health disparities, will achieve their optimal lifespan with the best quality of health in every stage of life,‿ and “Healthy People in Healthy Places‿ which states that “the places where people live, work, learn, and play will protect and promote their health and safety, especially those at greater risk of health disparities.‿
