Project Abstract:(i) The Rhode Island Executive Office of Health & Human Services (EOHHS), Ryan White HIV Provision of Care & Special Populations seeks to maintain and enhance its activities towards the “National HIV/AIDS Strategy (NHAS) 2022-2025” and improve the Continuum of Care outcomes in Rhode Island with the utilization of HRSA Supplemental Part B funding. The program would like to continue and improve upon its efforts towards prioritizing care and treatment HIV integrated activities in the state. This will enable the program to continue to achieve positive health outcomes across the HIV Continuum of Care, improve viral suppression, increase case manager-client communication to provide an overall client centered approach to well-being. Rhode Island would like to continue and further expand its technical relationship with AJ Boggs, it’s current ADAP host server for Lifia to develop three (3) additional areas of resources for PLHIV with the creation of an: (a.) “Empowerment Index”, (b.) “Positive People” online Platform. (c.) Expand upon the technical integration between Clinkly ai (formerly TAVIE Red) and AJ Boggs to streamline client eligibility processes within Rhode Island’s Lifia ADAP platform which is hosted by currently by AJ Boggs. The “Positive People Platform” will have a suite of tools embedded within the platform to enable social support and resources surrounding reducing stigma and gain empowerment among communities of people living with HIV (PLHIV). The online “Empowerment Index” will highlight best practices by providing examples and guidance to service providers with the use of an accountability tool for the communities served.) The expansion of the Clinikly ai (formerly TAVIE) will enable the participants to take control of their health and emotional well-being. Rhode Island has moved over to an annual recertification process based upon HRSA’s PCN 21-02 but will requires a mid-term check-in as to maintain payor of last resort. This expansion allows the user to take control and participate in their health outcomes, tap into available resources (medication reminders, bus schedules, agency portfolios of offered services, emotional well-being etc.) and enhance communication with their agency case manager. In addition, this also reduces the burden to case managers with the mid-term check-in as this will be done autonomously through the Clinkly ai app and case managers will receive notification of the clients that have submitted their mid-term check in. Beyond the forementioned endeavors, Rhode Island seeks to continue its current endeavors which are utilized with the current HRSA Supplemental grant for: 1.) Continued ADAP medication formulary expansion 2.) Health insurance premium assistance for ADAP sponsored family plans 3.) Further enhancement of the Lifia ADAP/RI-FAB platform to streamline data analysis, increase capabilities within the case manager dashboards for client recertification and eligibility processes. The program would like to formally request a funding amount of $1,125,650. The continued supplemental funding will benefit and enhance Rhode Island’s viral suppression and service delivery by: (i) increasing access for PLW/A to obtain non-antiretroviral medications with the expanded ADAP formulary for PLWH/A (ii) Increase access to family health insurance premium assistance for under and non-insured clients (iii) Provide expanded resources for referral of health care & support services either remotely or in-person via Ryan White funded agencies. This will provide streamlined communication for clients and agency resources in the event of an unforeseen change in the health environment which may affect continued achievement of viral suppression and maintenance of HIV care and treatment.
