Mary Ochsner, Project Director 804-443-6286 Mary.Ochsner@brtava.org Website: www.brtava.org Grant Funds: $1,750,000.00 Bay Rivers Telehealth’s Behavioral Health Primary Care Project (BH-PCP) addresses the need for telehealth-enabled behavioral health services in the primary care setting and the need to increase provider knowledge of telehealth through continuing education. The project focuses on 11 counties in rural Virginia including: Counties of Lancaster, Northumberland, Richmond, and Westmoreland (Northern Neck-region), Essex, Middlesex, King William, King and Queen and Mathews County (Middle Peninsula-region), Northampton and Accomack (Eastern Shore- region), with a combined population of 150,028. The ultimate outcome for this project is a reduction in the rates of morbidity and mortality related to the effects of mental illness, behavioral issues and substance use, as the result of lowered barriers to access to behavioral health services and services which address social determinants of health. The Community Health Workers (CHW) will be located primary care offices to create connections between patients and healthcare providers, help patients navigate healthcare and social service programs, collect data and relay information to stakeholders, provide screenings and referrals. They will refer patients to mental health providers for services designed to address mental illness, prevent substance abuse and provide early interventions, which will be received using telehealth. In addition, the project will deliver telehealth training to providers.
