On behalf of participating health centers (PHCs), Aliados Health is excited to pursue FY 2025 Health Center Controlled Network (HCCN) cooperative agreement funding provided by the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA). Aliados Health is a consortium of health centers with a mission to strengthen the capacity of member health centers to provide comprehensive, integrated care that improves health and health equity in their communities. Through leadership in advocacy, informatics, outreach, enrollment and population health, our work ensures that health center members have the support and resources they need to offer consistently high-quality clinical care and effective care coordination. Aliados Health HCCN is a leading existing PHC network with a 17-year proven track record and infrastructure to effectively support PHCs. Our FY 2025 HCCN proposal represents an expansion of our HCCN network from 15 to 20 PHCs and 13 to 16 counties in Northern California. The 20 Aliados Health HCCN PHCs serve over 423,900 patients, including over 291,200 Medicaid and 57,200 uninsured. Forty percent of patients are Latino, 36% are best served in a language other than English, over 52% have incomes under 100% of poverty, and 5% have incomes over 200% of the Federal Poverty Level. Several PHCs are the only source of primary care in their communities, with some covering significant rural communities. Most of the PHCs in our network serve the majority of the uninsured patients in their individual geographic region. Aliados Health will build upon advancements made during our FY2022 HCCN program where we supported PHCs to utilize health IT and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care, focused on clinical quality, patient-centered care, and provider and staff wellbeing. We will leverage FY2025 HCCN funding to meet PHCs’ diverse needs and support PHCs to leverage health information technology (IT) and data to enhance how they deliver affordable, accessible, high-quality primary care with a focus on data management and analytics, interoperability of systems and digital health tools and Uniform Data System (UDS) UDS+ implementation. In addition to the required objectives, Aliados Health HCCN will focus on elective objectives in the areas of Strengthening Cybersecurity Support and Artificial Intelligence (AI). Aliados Health HCCN will collaborate with several partner organizations to leverage expertise and services in a complementary and cost-effective manner to enhance HCCN activities. We will also continue to broadly disseminate promising practices that are developed through this HCCN cooperative agreement. Aliados Health HCCN is uniquely qualified to continue to advance the use of health IT and data at PHCs to deliver high-quality, culturally competent, equitable, and comprehensive health care to the communities they serve.
