Health Center Controlled Network - For over 20 years, the Public Health Management Corporation (PHMC), a non-profit public health institute and Section 330-funded health center organization located in Philadelphia, PA. has provided comprehensive primary care services to vulnerable populations. PHMC is the grantee of record for the Keys to Quality Health Center Controlled Network (K2Q HCCN) (current grant #U86CS45893), a network of 11 Participating Health Centers (PHCs), and is the applicant submitting on behalf of the K2Q HCCN. The National Nurse-Led Care Consortium (NNCC), a 501(c)3 non-profit organization and HRSA-supported national training and technical assistance provider (NTTAP) since 2008, administers the K2Q HCCN on behalf of the governing board, Keystone Community Health Alliance (KCHA). The governing board is majority controlled by PHCs who are Health Center Program (H80) award recipients, is independent of its constituent PHC boards, and has control over the fiscal and strategic direction of the network. Since 2013, the K2Q HCCN has provided high-quality, customized health information technology (HIT) training, education, support, and resources to its members. The network looks to extend and expand these services as PHCs focus on HIT optimization to deliver affordable, accessible, high-quality primary care services. Through a culture of collaboration and knowledge sharing, a “no wrong door” approach, and a proven ability to focus on individual and collective needs, the network will build upon advances made over the past twelve years, to further support PHC performance improvement and enhance technology capabilities to meet HRSA network objectives. Our 11 PHC members consist of 10 H80 health centers and 1 look-alike, are located across four different states, and use five different ONC-certified EHR platforms. They operate sixty clinic sites, serving over 140,000 unique patients per year. Our PHCs include nurse-led health centers, co-applicant health centers, special populations, rural and urban health centers, and are reflective of the diverse nature of the HRSA Health Center program. The K2Q HCCN selected network wide objectives and developed targeted activities for this application based on PHC needs identified using internal program data, a network-wide needs, assessment, UDS data, and in accordance with lessons learned over the past twelve years. PHC members have set quantifiable goals related to data management and analytics; interoperability and data sharing; UDS+ implementation; digital health tool optimization; and social risk identification and resource connections. Each member health center will receive a customized HCCN workplan, and a comprehensive needs and strengths assessment to inform the execution of work plan activities within 90 days of grant initiation. Individual activities within the K2Q HCCN workplans will be customized to address specific needs and strategic priorities of PHC members while continuing to support overall programmatic objectives and goals. K2Q HCCN provides individualized training and technical assistance along with peer-to-peer learning collaboratives, quarterly quality improvement meetings and monthly check-in calls to support attainment of objective goals. PHCs will receive specialized training and technical assistance, as well as project management support from HCCN staff members with relevant issue-area expertise, as well as support from third-party vendors, when necessary. The following application package for FY25 HCCN funding through the Health Resources and Services Administration will lay out how the K2Q HCCN’s concierge approach to training and technical assistance meets the need of PHCs through responsive services, in collaboration with stakeholders from across the healthcare landscape, with demonstrated outcomes and fiscal responsibility.
