Health Center Controlled Network - The South Carolina Health Center Controlled Network (SCHCCN) is composed of 18 Federally Qualified Health Centers (FQHCs), (2) two Look-alike, and the South Carolina Agriculture Worker Health Program (SCAgWHP), collectively serving 88% of the unique patient population across 39 of the 46 counties in South Carolina. As a trusted leader in health care innovation, the SCHCCN strives to support its members in optimizing clinical quality, advancing health IT, and improving operational efficiencies, aligning with both state and national health care priorities. During the upcoming project period, the SCHCCN will focus on activities that complement the following key objectives: 1. Data Management and Analytics: The SCHCCN will prioritize increasing the percentage of Participating Health Centers (PHCs) that effectively advance and optimize clinical, financial, and operational data to improve patient care and operational efficiency. By implementing enhanced data-driven strategies, health centers will gain valuable insights into their performance and be able to address both patient needs and organizational goals. This objective will support continuous improvements in clinical quality, health outcomes, and organizational sustainability. 2. Interoperability and Data Sharing: The SCHCCN aims to increase the percentage of PHCs that strengthen bi-directional interoperability with health care providers and community-based organizations. By fostering seamless communication across systems, health centers will be equipped to enhance health outcomes, improve patient experiences, and reduce provider burden. The SCHCCN will work closely with PHCs to adopt standards-based health IT systems that facilitate efficient information exchange, thus promoting holistic and coordinated care. 3. UDS+ Implementation: A central priority will be increasing the percentage of PHCs that submit disaggregated patient-level data in their UDS+ reports each calendar year as required by the Health Resources and Services Administration (HRSA). The SCHCCN will continue to provide training and technical assistance to PHCs to enhance their data collection, submission processes, and overall data governance practices, ensuring compliance with UDS+ reporting standards 4. Value-Based Care (VBC): The SCHCCN will focus on increasing the percentage of PHCs that leverage data to update their operational, financial, and clinical processes in preparation for, or participation in, value-based care models. By encouraging PHCs to integrate health IT systems that support value-based care, this objective seeks to enhance the patient and provider experience, improve health outcomes, and reduce health disparities. Through these efforts, PHCs will be better positioned to transition from fee-for-service to value-based care models, contributing to more sustainable health care delivery systems. 5. Artificial Intelligence (AI): Recognizing the potential of AI to revolutionize health care delivery, the SCHCCN aims to increase the percentage of PHCs that adopt AI technologies to optimize clinical, financial, and operational data. By integrating AI into data-driven strategies, PHCs can further advance their goals of improving clinical quality and health outcomes while minimizing administrative burdens and optimizing resource allocation. 6. Cybersecurity Support: With the increasing importance of data protection in healthcare, the SCHCCN is committed to bolstering cybersecurity capabilities across South Carolina’s community health centers. Our objective is to increase the percentage of PHCs that develop and implement robust assessments and advanced techniques to protect against cyber threats to health center data. By enhancing cybersecurity measures, PHCs can secure patient information, safeguard operational continuity, and build resilience against potential data breaches.
