Health Center Controlled Network - Neighborhood Health Care Network (NHCN) dba Federally Qualified Health Center Urban Health Network (FUHN) was first awarded HCCN funding in 2016 under Grant #H2QCS30378. The Federally Qualified Health Center Urban Health Network (FUHN) is a 501(c)3 nonprofit of federally qualified health center members (FQHCs) including one FQHC look-alike and eleven FQHCs. These health centers are predominantly based in the Twin Cities, Rochester and Mankato metro areas. FQHCs are community-based organizations that provide comprehensive primary care and preventive care, including medical, dental, and mental health/substance abuse services to people of all ages and backgrounds, regardless of their ability to pay or health insurance status. FQHCs are a critical component of the U.S. healthcare safety net. In 2012, 10 founding clinics formed an Integrated Health Partnership (IHP, a Medicaid ACO Model) with the Minnesota Department of Human Services on Medicaid healthcare reform efforts. Through the initial arrangement within the IHP, the participating clinics achieved more than $26 million in savings for the state. In 2023, FUHN formed a subsidiary Clinically Integrated Network to house innovative payment arrangements with payers. Today, FUHN member clinics participate in a new value based agreement with a managed care organization for preventive care improvement and have plans to participate in a Medicare Shared Savings Program in 2025. The centers aspire to enhance the care delivered to patients through affiliation arrangements with health systems, greater data sharing and utilization, and shared vendor arrangements to reduce cost and/or improve revenue in order to stabilize operating performance and achieve long term sustainability. FUHN also serves as a Health Center Controlled Network (HCCN) with nine FQHCs and one FQHC-LAL participating. The other two FUHN members participate in alternate HCCNs today. FUHN supports its member clinics in leveraging health IT and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care to effectively participate in value-based care arrangements. FUHN's team provides data aggregation and analytic support to provide measures and focus on improvements in clinical quality, patient-centered care, and provider and staff well-being. Together FUHN and the participating clinics have a shared vision for delivering high-quality healthcare for under-served populations in the Twin Cities and surrounding communities. FUHN has a strong and active board, and its committees and work groups collaborate on shared strategies and tactics in an organized manner which leverages the strengths of each health center member.
