The mission of IPHCA is to champion the development and delivery of accessible, community-driven quality health care. As a 501(c)(3) membership organization, IPHCA represents the Health Center Program (H80) awardees, Health Center Look-Alikes, and State-funded Community Health Centers in Indiana (herein after referred to as “health centers”). IPHCA collaborates with the Health Resources and Services Administration (HRSA), current and prospective health centers, state agencies, community partners, and related stakeholders. In 2016, IPHCA established the Indiana Quality Improvement Network (IQIN) as a HRSA-funded Health Center Controlled Network (HCCN). IQIN has allowed IPHCA to leverage additional staff, partnerships, and funding opportunities to support the advancement of IQIN Participating Health Centers (PHCs) in their use of health information technology (IT) and data to support their efforts toward improved quality, population health strategies, and value-based care and payment models. In the current project period, IQIN is comprised of 33 PHCs across Indiana and is governed by the HCCN Committee of the IPHCA Board. Since April 2024, IQIN has identified the Quintuple Aim of Patient Experience, Population Health, Reducing Costs, Care Team Well-Being, and Health Equity as the “North Star” to guide the efforts of the network. IQIN works to continually advance PHCs toward the Quintuple Aim in collaboration with the Bureau of Primary Health Care (BPHC) and HRSA. The purpose of FY 2025 Health Center Controlled Networks (HCCNs) cooperative agreement funding is to support HCCNs, which are networks of participating health centers (PHCs) work collaboratively to leverage health information technology (IT) and data to enhance how they deliver affordable, accessible, and high-quality primary care. During the Project Period from August 1, 2025, to July 31, 2028, IQIN will propose and implement activities that address specific health center T/TA needs in the following areas: 1. Data Management and Analytics, 2. Interoperability and Data Sharing, 3. UDS+ Implementation, 4. Additional Value-Based Care (VBC), and 5. Digital Health Tools. Based on the 37 PHCs who have signed Memorandum of Agreements with IQIN for the proposed project, IPHCA requests support in the amount of $1,205,000 annually for the allowable and reasonable costs allocable to this HCCN Cooperative Agreement.
