Amount of Funding Requested: $705,000 per year. INConcertCare, Inc. (INCC) has served as a health center controlled network (grant number H2QCS25639) for 12 Federally Qualified Health Centers (FQHCs) in Iowa since 1997. Building on its mission-driven success, in 2024, INCC members provided care to 241,126 patients at 77 service delivery sites and 34 school-based clinics. This funding opportunity aligns with the following needs of INCC participating community health centers: [Data Management & Analytics] Our network strives to ensure that all our participating health centers are able to leverage Health IT and data to deliver high quality, culturally competent, equitable and comprehensive primary care to all patients who walk through the doors of our health centers. Member community health centers continue to expand value-based care activities in response to market changes. INCC’s analytics and reporting solutions include business intelligence, population health management, and data visualization tools. These tools will help supply care gap information for preventive services and screenings, operational and performance management data, and analytic insights to support health centers in optimizing patient outcomes effectively and efficiently. In partnership with IowaHealth+ and the Iowa Primary Care Association, this funding will enable Iowa community health centers to feed meaningful healthcare data to health center care teams to support evidence-based practices. [Interoperability of Systems & Digital Health Tools] INCC’s mission is to enhance community health centers’ capacity to care. The majority of INCC health centers have moved to Epic as their EHR in the last two years. Work to optimize and leverage the power and interoperability of the Epic platform will continue throughout this period. Listening sessions and needs assessment data show that patients’ expectations are changing regarding access to their data and records, as well as access to eHealth services such as telehealth visits and remote patient monitoring. INCC knows that technology has a large role to play in making care accessible to our patients and we are striving to reduce technical barriers to allow for better care. This funding opportunity will enable INCC to increase digital engagement solutions for patients in multiple languages, enhance the sharing of data, and implement new technologies that will help remove barriers to care. INCC will assist with connecting to partner systems to share data, reducing duplication of services. INCC will also work with member health centers to establish safe, fair, equitable, and informed artificial intelligence practices. [Uniform Data System Modernization (UDS+) Implementation] INCC has provided technical assistance on UDS to our member health centers throughout our history. As the UDS Modernization Initiative moves forward, INCC will support our PHCs with electronic submission of de-identified patient level data. INCC knows that UDS modernization is a continuous effort to improve UDS content, quality, and reporting procedures. These efforts focus on accuracy of clinical documentation, as well as reducing reporting burden and improving data quality to better evaluate community health center services and outcomes. UDS+ data submissions to HRSA will be automated and require more support and coordination with health center HIT vendors, and we expect that requirements for submissions may change over time. As the Iowa network, INCC plays a key role in assisting our health centers with submitting patient-level data as required by HRSA and building on those minimum reporting requirements during the period of performance.
