(HCN) is pleased to propose Phase V of the Quality Transformation Empowering Community Health (Q-TECH) program to support participating health centers in leveraging Health Information Technology (HIT) to enhance value-based care, clinical quality, and patient outcomes. With support from HRSA, Q-TECH V aims to equip 40 health centers across 13 states and territories (CA, FL, NM, MO, MT, PR, RI, KS, NC, HI, TX) with innovative, cost-effective HIT solutions that facilitate enhanced patient engagement, population health management, data interoperability, and advanced analytics capabilities. Through a structured approach, Q-TECH V will help centers implement tailored data-driven practices that increase efficiency and improve clinical and operational practices, allowing health centers to better meet the needs of their unique patient populations. The program focuses on core objectives such as: •Data Management and Analytics: Enabling centers to integrate clinical, financial, and operational data to inform quality improvement and streamline operations. •Interoperability: Expanding bidirectional data sharing between healthcare providers and community-based organizations to improve care coordination, reduce provider burden, and minimize care gaps. •UDS+ Implementation: Assisting centers with compliant, efficient submission of disaggregated patient-level data in UDS+ reports as required by HRSA. •Cybersecurity: Strengthening security measures and data privacy for patient and operational data through governance and policy development. •Digital Health Tools and AI Integration: Supporting the use of digital tools and the integration of safe, equitable AI practices within Epic EMR to improve health outcomes, reduce disparities, and drive clinical decision-making. HCN will provide PHCs with continuous guidance from experts, quarterly progress reviews to ensure milestones are met, and opportunities for peer collaboration across the network to share best practices and learnings. Our partnership with Epic will further support PHCs in adopting tools such as Care Everywhere for seamless data interoperability and Happy Together to integrate external records into local EMRs, enhancing patient engagement and data accuracy. As Q-TECH V progresses, HCN will strengthen PHCs’ ability to engage in effective referral management, participate in health information exchanges, and support efficient data-sharing workflows—all critical to improving health outcomes and expanding value-based care participation. By fostering a cohesive, technology-enabled environment, HCN’s Q-TECH program aligns with HRSA’s mission to advance health equity, reduce disparities, and empower FQHCs to deliver high-quality, coordinated care to underserved populations nationwide. Through this comprehensive approach, Q-TECH V will position PHCs for sustainable improvements in care quality and operational efficiency, ultimately benefitting the communities they serve.
