The 11 DHN participating health centers (PHCs) serve areas of need throughout the United States, including: CA, DE, MN, IL, OH, and SC. PHCs have sought the support of DHN for its reputation as a hands-on HCCN, which supports individualized health center needs for utilizing and optimizing health information technology (HIT) and data to deliver high-quality, culturally competent, equitable and comprehensive primary health care, with a specific focus on improvements on data management and analytics, interoperability of systems and digital health tools, and Uniform Data Systems (UDS+) modernization implementation. DHN’s unique approach to analytics and data management has drawn enthusiasm from PHCs, who continue to rely on DHN for data tracking and quality improvement support. DHN works directly with PHC staff to configure, build, and present data that is tailored to each PHC’s unique needs. DHN will further expand on this vital analytics support by embarking on a data management initiative. DHN will conduct an analytics assessment for each PHC to identify gaps in data strategies which will be used to inform the development of a data management plan. DHN’s innovative analytics approach will help PHCs develop a plan that leverages their data in a meaningful way to advance clinical quality, health outcomes, and operations. DHN’s experiential learning program provides training to PHCs, both virtually and in person, to promote health literacy. Training sessions improve the operational and clinical processes in HIT systems that support value-based care (VBC) initiatives to enhance health outcomes. Developed through data driven insights, DHN’s program helps PHCs streamline documentation and standardize data to promote informed decision making. DHN supports VBC workflows amongst PHCs by delivering personalized customizations to improve ease of data capture for imperative health initiatives. All educational resources are housed in DHN’s learning management system (LMS). DHN will leverage our robust training program and LMS to train PHC staff on data sharing platforms to promote interoperability and data sharing. DHN will encourage bi-directional data exchange through the adoption of a Quality Health Information Network or Diagnostic Orders and Results Network. We will provide regular updates on healthcare legislation, regulation, and HIT vendor enhancements to strengthen regulatory, privacy, and cybersecurity compliance. DHN will facilitate the submission of disaggregated patient level data through the UDS+ FHIR API by providing educational resources and support. We will coordinate with vendors and PHCs to produce a guide based on 2025 submissions and will leverage those insights to train PHCs on UDS best practices. DHN will guide PHCs through reporting platform configuration and utilization and partner throughout the submission process to ensure the successful transmission of disaggregated, patient-level UDS data. DHN is committed to data integrity and security and will promote the introduction of artificial intelligence (AI) tools that align with fair, appropriate, valid, effective, safe (FAVES) AI principles by helping PHCs develop and implement an AI risk management framework. DHN will partner with vendors and PHCs to evaluate AI solutions and will foster the implementation of an AI risk management framework to support institutional readiness and give PHCs the structure they need to maintain AI practices.
