Health Center Controlled Network - PROJECT ABSTRACT SUMMARY CFDA: 93.527 FON: HRSA-25-018 Applicant Name: Coalition of Orange County Community Health Centers (COCCC) Project Title: Orange County Partners in Health (OCPH) Health Center Controlled Network (HCCN) Narrative response (4,000 character limit): The Coalition of Orange County Community Health Centers (COCCC), an established network of 21 independent, 501(c)(3) non-profit health center organizations throughout Orange County, California, has been operating a robust Health Center Controlled Network (HCCN) since 2016 with support from HRSA. COCCCC is the fiduciary intermediary of the HCCN, which is governed and operated by the Orange County Partners in Health (OCPH). The HCCN includes 10 Participating Health Centers (PHCs), of which 8 are 330-funded Federally Qualified Health Centers (FQHCs) and 2 are FQHC Look-Alikes. The PHCs have a combined total of 43 clinical sites serving diverse populations throughout Orange County. OCPH HCCN staff recently conducted a detailed needs assessment of the 10 PHCs. The findings of the needs assessment have informed this proposal. OCPH and its PHCs have a strong track record of leveraging health information technology and data to enhance their delivery of affordable, accessible, and high-quality primary care. A FY25 HCCN Cooperative Agreement will enable OCPH to strengthen its support for PHCs in the following areas: data management and analytics (Objective 1); interoperability of systems and digital health tools (Objective 2); Uniform Data System Modernization (UDS+) implementation (Objective 3); value-based care (Objective 4); and social risk factors (Objective 5). OCPH is poised to continue to work through the OCPH to diligently collaborate with PHCs to improve the quality of healthcare provided to more than 125,000 of the most vulnerable and medically underserved residents of Orange County, California. The PHCs are accustomed to collaborating within the OCPH HCCN to share best practices to achieve common goals and will continue to engage in this peer-to-peer learning framework.
