Hospital Preparedness Program Cooperative Agreement - The Chicago Department of Public Health (CDPH) is committed to the vision of an equitable, safe, resilient, and healthy Chicago. The Hospital Preparedness Program (HPP) aims to enable the City of Chicago’s healthcare delivery system to prepare for, respond to, and recover from emergencies and disasters. Through the Chicago Healthcare System Coalition for Preparedness and Response (CHSCPR), Chicago’s Healthcare Coalition (HCC), the HPP at CDPH coordinates programmatic activities, enhances partnerships, and advances preparedness planning for hospitals, public health, and healthcare partners (i.e., long-term care centers, dialysis centers, Federally Qualified Health Centers (FQHCs), and Emergency Medical Services (EMS) providers.). Over the next five-year Cooperative Agreement performance period, HPP funding will support exercises, training, pharmaceutical caches, supplies, equipment, plan development, IT systems, and interoperable and redundant communications—all of which contribute to the City of Chicago’s ability to effectively respond to and recover from public health emergencies. Capability development of the HPP throughout the five-year cooperative agreement will ensure that both public health and the healthcare system are prepared and coordinated during response efforts. CHSCPR has operated in its current model since 2009. The HPP will continue to co-lead the CHSCPR to meet all ASPR grant requirements. CDPH will continue to build upon lessons learned from drills, exercises, and real-world events to inform plan revisions, identify gaps within capabilities, and maximize both human and financial resources. The HPP at CDPH is confident that a standard of excellence will be maintained during the new five-year project period and within Budget Period 1, while fulfilling all ASPR cooperative agreement requirements.
