Regional Telehealth Resource Centers - Proposed Service Area: South Central Region – Missouri, Kansas, and Oklahoma Target Populations: Rural and frontier communities; medically underserved individuals; low-income families; veterans; seniors; non-English speakers; school-aged youth; and individuals with chronic conditions or disabilities. Cape Family Medical Clinic, LLC (CFMC) proposes to lead the South Central Regional Telehealth Resource Center (SCRTRC). This comprehensive regional hub will provide telehealth training, technical assistance (TA), program evaluation, and knowledge dissemination across Missouri, Kansas, and Oklahoma. Drawing on CFMC’s long-standing commitment to rural health and its proven telehealth infrastructure, including school-based virtual care, remote chronic disease management, and mobile telehealth deployments—this RTRC will build capacity among healthcare providers, public health entities, and community-based organizations throughout the South Central Region. SCRTRC will deliver core services aligned with HRSA’s goals, including tailored TA and provider education, support for telehealth integration into clinical workflows and health IT systems, consumer education initiatives, cybersecurity consultation, and evaluation of emerging telehealth models. Through partnerships with rural health clinics, schools, libraries, and community organizations, the center will promote digital health equity, reduce care disparities, and expand access to quality virtual healthcare across underserved and hard-to-reach populations. With a deep understanding of the unique geographic, economic, and cultural dynamics of the South Central Region, CFMC will ensure that the SCRTRC empowers regional stakeholders to implement sustainable, secure, and patient-centered telehealth solutions. The center will actively collaborate with other RTRCs and HRSA to share best practices and support nationwide efforts to modernize healthcare access through innovation and inclusion. CFMC meets all three qualifications and respectfully requests consideration for all three funding preferences as defined in the NOFO for HRSA-25-042. for funding preferences: 1) CFMC has a record of success in providing telehealth services to rural areas, medically underserved areas, and medically underserved populations. 2) CFMC has a demonstrated record of collaborating and sharing expertise at the local, state, regional and national level. 3) CFMC has a record of providing a broad range of telehealth services.
