Guidiville Self Governance Cooperative Planning Project - The Guidiville Indian Rancheria seeks funding through the Indian Health Service (IHS) Tribal Self-Governance Planning Cooperative Agreement to conduct a comprehensive two-year planning project focused on strengthening Tribal health services and evaluating readiness to participate in the IHS Tribal Self-Governance Program. Since 1991, the Tribe has operated a Community Health Program that provides basic health services, including Community Health Representative (CHR) services, transportation assistance, health advocacy, referrals, substance abuse prevention, and limited medical bill assistance. While these services have benefited Tribal members for more than three decades, the Tribe recognizes the need to modernize and expand its health care delivery system to address emerging health priorities, health disparities, and evolving community needs. Through this project, the Tribe will assess its organizational, administrative, legal, and financial readiness for Tribal Self-Governance and identify opportunities to expand its Programs, Services, Functions, and Activities (PSFAs). A primary goal is to increase Tribal control over health program design and administration, allowing services to better reflect Tribal culture, traditions, and community priorities. The Tribe is particularly interested in exploring opportunities to incorporate traditional medicine, cultural healing practices, spiritual guidance, and Indigenous wellness approaches into future health programs. Project activities will include organizational assessments, community outreach, surveys, listening sessions, community scoping meetings, and a Community Health Needs Assessment. The Tribe will engage Tribal leadership, staff, Elders, youth, and community members to identify health priorities, service gaps, barriers to care, and opportunities for future program development. The project will also evaluate potential expansion of services such as behavioral health, diabetes prevention and management, youth wellness programs, elder services, health education, preventive care, and culturally based wellness initiatives. The Tribe will develop a comprehensive Self-Governance Readiness and Strategic Roadmap outlining priority PSFAs, organizational capacity needs, implementation considerations, and recommendations for future health service expansion.
