The Coushatta Tribe of Louisiana is planning to negotiate a Title V compact with IHS and is requesting this cooperative planning grant to fund the planning phase of the process. - The Coushatta Tribe of Louisiana (“Tribe”) is a federally recognized Indian tribe with approximately 960 members. The Coushatta people have called Southwest Louisiana home for more than a century. Today, most Tribal members live in Allen Parish, just north of the town of Elton, Louisiana, and east of Kinder, Louisiana. The Tribe regained federal recognition in 1973, and in 1995, it opened the Coushatta Casino Resort, which currently employs nearly fifteen hundred (1,500) people. The Tribe opened the Coushatta Health Department (“CHD”) in the mid-1970s. In 2015, the Tribe opened the Coushatta Family Medical Center & Pharmacy, which was rebranded as Koasati Medical Center and Pharmacy (“Koasati”) in 2025. CHD and Koasati are collectively referred to as “Clinics” throughout the rest of this summary. CHD serves Tribal members via its clinic and pharmacy. Koasati serves both Tribal members and employees of the Coushatta Casino Resort, as well as their dependents. Currently, the Clinics provide direct medical outpatient services; behavioral health services; purchased and referred care; health and community education and outreach services; health promotion and disease prevention services; elder care programming; maternal and child health programming; and pharmacy services. The Tribe is interested in negotiating a Title V compact with the Indian Health Service ("IHS"). The beginning of this process includes a planning phase in which the Tribe completes the following objectives: 1) researching and analyzing the IHS budget to gain a thorough understanding of how funding is distributed at all levels of the organization; 2) evaluating the current level of health care services provided to make an informed decision on its capacity to take on new or expanded programs; 3) establishing a process to identify the programs, services, functions, and activities ("PSFAs") it plan to take on or expand; 4) determining which PSFAs it may want to take on or expand; 5) establishing a process to identify PSFAs and associated funding that it could incorporate into current programs; and 6) determining what share of each PSFA it wants to administer. The Tribe is requesting the funding to complete the Title V planning phase. This planning grant will allow the Tribe to assess community needs and determine which PSFAs would most benefit the Tribe if taken over from IHS. The Tribe hopes to use the funding from this cooperative agreement to hire a consultant and upgrade its electronic health record systems so that it is able to complete the planning phase, progress to Title V negotiations, and ultimately assume PSFAs. Taking on PSFAs will greatly enhance the Tribe’s ability to provide comprehensive healthcare services to its members, employees, and community at large. Taking on PSFAs means the Tribe can not only address gaps in its current healthcare coverage but also bolster programs it already provides. Upgrading its electronic health record ("EHR") systems will allow the Tribe to integrate care coordination among its Clinics, which will allow for greater continuity of care for patients. Taking on PSFAs will allow the Tribe to build capacity locally and potentially train Tribal members in preventive and clinical care protocols. Additionally, the Tribe will be able to direct additional resources toward preventive care delivered through a culturally specific lens, improving health outcomes for the entire community.
