Capacity to serve rural underserved populations CDS has collaborated with all network partners in past HRSA or Minnesota Department of Health initiatives and together have collaborated with over 50 rural counties, townships, mental and community health, school districts, SUD/OUD prevention, treatment and recovery organizations, tobacco cessation and vaping prevention projects over the past decade. With over 100 years of service, CDS’ staff have extensive experience and expertise in implementing collaborative, multifaceted health interventions across rural regions. CDS remains the only community dental service
s agency in Minnesota dedicated exclusively to providing oral health treatment and public health training to increase services to rural, vulnerable children, pregnant women and people, and families. CDS provides a full range of treatment and workforce training in 51 Minnesota counties, serving over 20,000 rural patients annually. Need: Target counties are fully designated as HPSAs. Demand: Over 10,000 rural patients and 50 rural partners annually request CDS’ care. Objectives: CDS will create a Southwest Minnesota Network expanding clinical and public healthcare training programs across rural, Southwest Minnesota in 7 underserved counties. Outcomes: The rural public health workforce will be expanded by 90, and 25,000 additional low-income, at-risk community members will be served. This program will lead to an increase in health professionals in the workforce by 60-70%. Clinical Services Provided: The Network will directly increase comprehensive dental and medical services to low-income and underinsured children, pregnant people and adults of all ages across Minnesota by providing culturally appropriate training for providers. Actual Patients/Persons Served: 25,000 low-income community residents Expected Outcomes: The project will establish a Training Network and expand clinical and public healthcare training for 90 public health workers, who will serve 25,000 low-income, at-risk patients. Sustainability: Achieved via shared staffing, space, training, use of the Health Care Access Fund, use of mid-level providers, and investment of Critical Access Funding.
