Albany State University’s Community Partnerships for Youth Health Innovations has developed regional, national and international multi-disciplinary partnerships to create six ‘innovation hubs” to address the priority area, Expectant and Parenting Youth. The innovation hubs will serve 18 Southwest Georgia counties- which include some of the poorest, most distressed areas in the United States and will connect contiguous counties into Action Councils. The Action Councils will infuse capacity into each hub through training, technical assistance, collaboration and electronic networking. Locally-governed innovative projects using, trauma-informed, patient-centered approaches, will reduce the high rates of teen pregnancy, STIs and improve health for teens. Albany State University will serve as the fiscal grantee, provide faculty expertise, provide dedicated program staff, and coordinate community organizations and Radical Partnerships to bring resources to each hub. The project will use the Social Ecological and Social Cognitive theoretical frameworks to create multi-level, multi-factorial systems that will effect change throughout the region and culminate in positive outcomes. Ongoing planning, analysis, evaluation, and reporting will be used to document the success and profound impact of the project.
