With funding from the Office of Population Affairs, the Cabarrus Health Alliance (CHA) will improve equity and reduce disparities as a primary strategy to generate optimal health for adolescents age 15 – 19 years old. The Elevate program will focus on high risk populations within the geographic area of Kannapolis, located in Rowan and Cabarrus counties, North Carolina. The goal of Elevate is to reduce teen pregnancy, STI rates, and disparities by: a) replicating two effective programs, b) creating and sustaining supportive services that promote optimal health, and c) leveraging the systems that impact adolescent health to improve equity. Kannapolis students live in families that, when compared to national averages, have a lower rate of post-high school completion (39.5% lower), less health insurance coverage (31.2% less), and lower median household income (13% less). Students’ significant poverty is represented by the fact that 100% of Kannapolis City Schools (KCS) students receive both free breakfast and lunch. Teen birth rates are up to 86.5% higher than national rates; the largest disparities exist between teens living in Cabarrus County and those living in Rowan; and between Hispanic teens and non-Hispanic teens across both counties. The proportion of Hispanic teen pregnancies increased 23.73% in Cabarrus and 13.7% in Rowan between 2015 and 2017. In addition, Hispanic teens are having babies at a younger age than other races/ethnicities, indicated by an upward trend of births by Hispanic mothers 0-17 years old (from 30.6% of all births in 2015 to 39.1% in 2017). Trend data also shows that Kannapolis students are consistently at high risk of contracting chlamydia and gonorrhea. Black/African American youth in particular face alarming and disproportionate disparities in STI infections, with rates up to 215% higher than the national average. Elevate leverages CHA’s extensive experience and partnerships in adolescent health to create and sustain an environment that encourages positive youth development, student leadership, and responsible decision making, all within a race equity lens. In addition to traditional approaches of replicating evidence-based curricula for ninth grade and Hispanic students, CHA will provide culturally-appropriate components to best serve youth experiencing the greatest disparities through direct parent and student support services, community outreach, and systems-level change. A primary strategy of Elevate will be to leverage community-wide partnerships to build a systems of care approach around our target population while also instituting a new cultural norm around equity among this group of influencers. In partnership with KCS and racial equity contractors, CHA will initiate a Learning Collaborative to provide a forum elevating equitable and optimal health into the forefront of community practice. The Learning Collaborative will initiate a culture shift to mitigate risk factors disproportionately affecting minority youth, while highlighting their strengths and resilience. Systems-level stakeholders will maximize opportunities for cross-sector and cross-community partnerships that build consensus around medically-accurate, trauma-informed, and age-appropriate adolescent health Action Plans. Activities and findings will be communicated at the local, state, and national levels through robust dissemination designed to ensure the greatest collective impact on adolescent health can be achieved.
