The Hennepin County Public Health Department and Better Together Hennepin Initiative, a unit of local Hennepin County government in Minnesota, intends to improve the optimal health of adolescents and reduce teen pregnancy and sexually transmitted infections in six Hennepin County communities with the greatest needs and disparities. In order to achieve this goal, the program implements the Health Mentor Model in 11 public high schools and in partnership with community-based clinics. The project embeds sexual health educators, from local adolescent-friendly clinics, called Health Mentors into schools and uses a four-tiered approach to support the unique needs of these students, schools and communities. The model is responsive to the multi-dimensional needs of a community school environment and includes classroom education, small groups, one-to-one education and systems work to support safe and supportive environments. By working with schools and clinics, the Health Mentor Model Project (HMMP) operates in systems where measurable effect is possible; builds relationships and linkages with other systems that intersect; and puts pressure on key system levers to drive change forward toward optimal health. The project reaches 4,671 youth between the ages of 15 and 19 each project year with the replication of effective programs. A wide range of supportive services is available to the entire school population of 12,977 youth and reaches up to 600 parents annually.
