Centerstone's Healthy You(th) (HY) is a comprehensive, culturally/age-appropriate, medically accurate, trauma-informed Teen Pregnancy Prevention Program (TPP) to reduce teen pregnancy rates and existing disparities, and support optimal health for youth. HY will serve an unduplicated 7,920 youth (ages 12-19) in 20 counties in Tennessee (Years 1-3: 2,640 annually). HY’s focus population includes youth in areas with high teen birth/STI rates, culturally underrepresented, in/aging out of foster care, involved in juvenile justice system, and/or disconnected. HY’s focus population mirrors catchment area demographics: 49% male, 51% female, 71% white, 17% African American, and 7% Hispanic/Latino individuals. In 2018, more than 70% of the target population had a teen birth rate (ages 18-19) higher than Tennessee’s rate. HY will engage schools and residential settings in the provision of TPP education; enhance existing TPP services by providing trained experts evidence-based programs (EBPs)/curricula for teen pregnancy; offer activities that support increased access to youth friendly medical services; increase opportunities to engage/educate parents/caregivers; increase opportunities for youth to engage as learners, leaders, team members, and workers; increase youth/family and stakeholder voice in service implementation and evaluation; add technology enabled tools to address disparities in youth access to care, particularly for rural youth; and expand HY staff capacity to collect/report/use local data related to teen pregnancy/STIs/high-risk behaviors. HY’s EBPs include Power Through Choices, Choosing the Best Journey, and Positive Potential. HY will accomplish the following goals: 1) Implement/establish a comprehensive project that promotes equity in reaching optimal health, reduces teen pregnancy/STIs, and saturates communities with greatest need/disparities; 2) Develop/enhance a comprehensive infrastructure/ capacity to maintain a continuum of effective, innovative, and evidence-based HY services/ supportive services; 3) Improve health status and psychosocial outcomes among HY participants; 4) develop/expand parent/caregiver opportunity for involvement in/support of TPP 5) Develop/disseminate a documented service model for replication/adoption. HY will achieve the following measurable objectives: Achieve 25% saturation rate among identified populations within geographic boundaries; Establish/coordinate with a referral network of 15 youth-friendly service providers/organizations; Conduct culturally sensitive outreach education to 1,000 community stakeholders; Decrease health myths that influence beliefs/ behaviors in participating youth by 40%; Increase youth knowledge of HIV/AIDS/STIs/TPP by 50%; Increase youth confidence/ability to negotiate sexual situations by 35%; Support 100% of participants in need to identify/secure health-related services/supports; Provide 100% of parents/ caregivers TPP educational resources; and Offer parent/caregiver education sessions to 100% of school implementation sites. HY has secured memorandums of understanding and letters of support from partners dedicated to the project’s success.
