The Augusta Partnership for Children (APC), Augusta Campaign for Adolescent Responsibility and Equity (CAREs) is a multidimensional youth centered teen pregnancy prevention and adolescent health initiative that seeks to promote optimal health among youth residing in the socially and economically disenfranchised community of East Augusta. Using the Aligning Community Systems (ACS) framework, APC will facilitate the development of a youth-driven community action plan that will incorporate strategies and approaches informed by youth, systems leaders and community stakeholders to address the complex needs of vulnerable youth in East Augusta. Augusta CAREs will incorporate a number of strategies and evidence-based programs including the Love Notes EBP, in an effort to reach 1500 youth throughout the project life cycle.
