Project Summary Abstract Project Title: TRUST (Think, Relate, Understand; Self-efficacy to Thrive) Project Applicant Name: PRO Youth & Families, Inc. Address: 4625 44th Street, Sacramento, CA 95820 Contact: Clay Rojas, Project Director Contact Number: 916-576-3300 Contact Email: clayr@proyouthandfamilies.org Web Site Address: www.PROYouthAndFamilies.org The goal of the TRUST Project is to empower justice-involved youth, ages 12-18, in Sacramento and Placer Counties, California to increase self-efficacy to achieve optimal health, resulting in reduced rates of adolescent pregnancy and STIs. Community Need. The data indicates a clear and present need to address reproductive health in these geographical regions especially when one considers the racial/ethnic disparities in the data. Teen pregnancy rates for youth of color in economically disadvantaged neighborhoods, with high rates of justice-involved youth, are far exceeding national averages. Rates of STIs in Sacramento County are skyrocketing and are much higher than state and national rates. For example, Sacramento County chlamydia rates have risen 44% in the past six years and are 46% higher than the national rate – and are highest among young people ages 15-24. Worse still, we believe the statistics are even worse within the juvenile justice population – the target population for this project. Juvenile justice youth realize some of the worst health outcomes, have the least amount of family and community support, and be the most disconnected when it comes to relationships with a caring adult, family engagement, and opportunities for meaningful involvement. Target Population Reach. Through integration with current programs and systems serving justice-involved youth (including county probation offices, juvenile justice chaplaincy programs, and restorative justice programs), the project will reach 25% of our target population each year resulting in serving 1,875 youth over the three-year project period. Approach. Utilizing a systems-levels approach incorporating The Adolescent Health: Think, Act, Grow® framework, this project aims to address the disconnect by bringing together a collaborative network of experienced and community-trusted organizations to replicate Love Notes EBP, an evidenced-based teen pregnancy prevention program and provide coordinated support services designed to facilitate access to health services and to meaningful youth engagement opportunities. The project will also incorporate youth-led community engagement strategies to ensure youth and community participation in planning, implementation and evaluation.
