Behavioral Health Navigation (BHN) - Interfaith Medical Center (IMC) is proposing a program to expand substance use disorder treatment, behavioral health and HIV/hepatitis services for high risk populations (African-American and Hispanic/Latino men and women, YMSM, and MSM) who have substance use disorders (SUD) and/or co-occurring substance use and mental disorders and are living with or at risk for HIV/AIDS. The service area will be the New York City area of Central Brooklyn. IMC's program activities will include: Evidence-Based Practices (CLEAR) Choosing Life: Empowerment, Action, Results, Modelo de Intervencion Psychomedica (MIP) , and FIT (Focus on Integrated Treatment), SUD/Co-Occurring Disorders Treatment Services, HIV Testing and Case Management Services, and Viral Hepatitis Testing and Referral, including referrals and linkages to follow-up care and treatment. The program's goals are: reduce the negative impact of behavioral health problems; increase access to and retention in treatment for behavioral health conditions, reduce the risk of HIV; reduce new HIV and viral hepatitis infections; and increase the provision of linkage to HIV care. The program's objectives are to: reach 300 persons through targeted outreach and conduct GPRA, assessments, and develop individualized recovery plans, decrease substance use among at least 70% of those enrolled in MIP, provide substance abuse treatment services to 90 persons per year, decrease rates of substance use among 35% of participants, provide HIV tests to 250 persons per year, increase condom use among 80% of CLEAR participants, provide hepatitis screening to 250 persons per year, connecting 90% of those testing positive for VH to care within 30 days, connect 90% of those testing HIV-positive to care within 30 days, provide ongoing case management services to at least 50 HIV-positive persons per year.
