MILWAUKEE CONTINUUM SUPPORTIVE SERVICES PROJECT (MCSSP) - The City of Milwaukee and its four partnering agencies propose the Milwaukee Continuum Supportive Services Project (MCSSP). Through MCSSP, 60 homeless and chronically homeless veterans and chronically homeless non-veterans will receive targeted assistance in Year one (84 over the three-year grant period) to link them with integrated outreach, case management, housing, substance use and mental health treatment, benefits, and additional services. The proposed evidence-based practices that will be implemented are: Motivational Interviewing and Housing First. The Milwaukee Continuum of Care's supervision and coordination of the collaborative MCSSP ensures that it is linked to the City of Milwaukee's overall goals and work around ending chronic homelessness and supporting veterans. The objectives to be measured by the performance assessment include assisting participants in: 1) improving access to and engagement with services; 2) improving access to income and benefits; 3) improving level of functioning related to mental health and substance use; and 4) improving access to housing.
