Integrated housing, substance abuse/co-occurring treatment and support services - Buffalo Valley, Inc. proposes a project that will address a critical gap in access to integrated housing, substance abuse/co-occurring treatment, and support services in middle Tennessee for veterans who experience homelessness or chronic homelessness and non-veterans who experience chronic homelessness. The project plans to serve a total of 200 individuals during the life of the project: 60 in the first year and 70 in each of the subsequent two years. Strategies and interventions include implementing Housing First with Assertive Community Treatment (ACT) and Intensive Case Management (ICM) teams. The project will also utilize several evidence based practices including Integrated Dual Disorders Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, Illness Management and Recovery, Supported Employment, Stages of Change, Relapse Prevention Therapy, and Seeking Safety. The goals of the project are to: provide integrated, comprehensive services to 200 homeless individuals during the life of the project; increase access and retention in evidence-based integrated treatment programs; place homeless veterans and chronically homeless non-veterans in permanent housing with coordination of supportive services; and engage and connect eligible clients with mainstream resources.
