Santa Clara County of California will implement the Valley Healthcare and Housing for the Homeless (VH3) Project to serve 240 chronically homeless individuals and families, including veterans, who experience substance use disorders, as well as co-occurring mental and substance use disorders. The VH3 project offers placement into permanent housing and substance abuse treatment within a comprehensive system of integrated services that support recovery. The project has developed an Enhanced Screening, Brief Intervention, and Referral to Treatment (Enhanced SBIRT) program design to achieve the project's goals. The innovative Enhanced SBIRT Model expands and extends the traditional SBIRT approach to respond to the multiple health, behavioral health and other problems experienced by the population of focus. Other evidenced-based practices to be implemented include: Seeking Safety, Trauma-Focused Cognitive Behavioral Therapy, Twelve Step Facilitation Therapy, Motivational Interviewing, Stages of Change Model, Comprehensive, Continuous, and the Integrated System of Care Model. The Housing 1000 Community Care Partnership Stakeholder (H1K CCP) Group will be expanded to serve as the steering committee for the VH3 Project. The H1K CCP Steering Committee will continue to meet weekly to coordinate the activities and the address the challenges involved in achieving the county's goals for ending chronic homelessness.
