FEMMES Plus - St. Barnabas Hospital (SBH) is seeking to enhance its Finding, Engaging, Managing, Motivating, Educating and Supporting (FEMMES Plus) program, which targets women. The Program will be enhanced by co-locating HIV risk reduction counseling, HIV rapid testing, hepatitis (B and C) testing, and mental health services within its existing substance abuse programs. SBH's Department of Addiction Medicine, Infectious Disease/Designated Aids Center (DAC) and Fordham-Tremont Community Mental Health Center (Fordham-Tremont) will collaborate to deliver these services to at least 750 women during the three year grant period. SBH's FEMMES Plus Program Goals and Objectives are to: Provide rapid HIV testing, education and risk reduction counseling to women in SBH's existing substance abuse program base; Provide testing for hepatitis B and C; Provide patient navigation and case management; Co-locate clinicians from Fordham Tremont Community Mental Health Center to provide mental health screening and treatment services; Provide evidenced-based comprehensive treatment to women with co-occurring disorders (MI and Boston Consortium Model: Treatment of trauma among women with substance use disorder); Facilitate linkage of HIV and hepatitis (B and C) positive women to SBH's Department of Infectious Diseases and the DAC; Facilitate referrals to community services such as primary care, housing, social support and treatment adherence. The FEMMES Plus program will serve women who reside within the SBH's primary catchment area in the Bronx, which located within New York State's 16th U.S. Congressional District.
