Re-Entry and Recovery Services for Substance Abusers - The Gaudenzia DRRSA will ensure that substance abusers being released from incarceration and returning to Philadelphia have the opportunity for recovery, self sufficiency, and stability. The project will provide expanded and enhanced substance abuse, aftercare, and recovery support services for substance abusers and family services to family members and significant others. The project will focus on adult minority substance abusers and those with co-occurring mental health disorders (CODs), recurrent trauma experiences, multiple incarcerations, and those with or at high risk for HIV/AIDS. They will have served at least 3 months in a jail, prison, or a community correction center and will be within 4 months of community release. The Project staff will meet regularly with the state and local corrections and parole staff to provide pre-release collaboration, coordination, assessments, and re-entry planning. Staff will also provide post release case management, referral/coordination, mentoring/coaching, re- assessments, CBT seminars, re-entry plan updates, relapse prevention beds, and family services. A Steering Committee comprised of staff, consumers, and collaborators will have a vital role in developing, implementing, and evaluating the project. The overarching goal is that clients will achieve and maintain recovery, live a crime-free and substance-free life, and become self supporting or self sufficient. With measurable objectives the following goals are established over 3 years: 1) Provide comprehensive services for POF offenders; 2) Provide services for family members, and as appropriate, foster reunification; 3) Engage 12 partners in a Community Case Management Program and Steering Committee; 4) At least 80% of clients will complete prescribed treatment/services; and 5) At least 65% of clients will experience improved criminal justice, substance abuse, mental health, housing, educational, medical/health, and employment outcomes.
