Project Reality Hepatitis C Project - The purpose of this program this program is to prevent and reduce future health problems related to hepatitis C by providing easy access to hepatitis screening for at risk individuals undergoing medication assisted treatment (MAT) for addiction. The program will identify people with hepatitis C infection and increase follow through with primary health care for monitoring and treatment. The targeted population for the program focuses on injecting drug users, recently/previously incarcerated individuals, veterans, patients in between the ages of 48 - 68 years of age , persons of color and men who have sex with men (MSM).The goals and strategies of the this proposed one year program are to: provide hepatitis risk/prevention education to all patients served; provide free rapid hepatitis C antibody testing on site with increase screening of among at risk minorities; enhance and facilitate documentation of successful referral services to primary care; and to address health care disparities in at risk individuals served.
