BLUEPRINT TO LIFE 2.0 - Community Bridges Inc. (CBI) is applying for funds through the Recovery Community Service Program (RCSP) as a facilitating organization (FO) to provide peer-based outreach and ongoing supportive care to homeless individuals who have untreated substance use and mental health disorders. The proposed service, entitled "BluePrints to Life 2.0", focuses its efforts on a more chronic, at-risk (health, mental health, substance abuse) population of individuals who experience an overuse of crisis and emergency resources as well as face a higher risk relapse, barriers to supportive services, long term hospitalization, and/or premature death. Individuals served will primarily be homeless adult men and women between the ages of 31-59 who have untreated, co-occurring disorders and have been identified to have had multiple community-based, crisis service admissions. Of those served, it is estimated that 55% will be White, 7% Native American, 19% African American, and 2% Asian. Just over 17% of the individuals to be served will identify their ethnicity as Hispanic. It is estimated that over the lifetime of the project, between approximately 95 and 100 individuals will be served.
