Project is aftercare intervention for those completing inpatient/intensive outpatient substance use disorder treatment (tx). Based on a cognitive-behavioral model, project will use technology to support aftercare with email/text message appointment/self-help support group reminders, along with series of social reinforcers (letters/certificates/small non-cash awards to encourage aftercare participation). Participants will receive info re: free online support/web-based resources to aid in aftercare program. Project undertaken through partnerships with 6 substance abuse tx providers throughout WV. Continuing care for substance use disorder strongly related to positive tx outcomes, but participation rates are low. Use of contracts/ prompts/CPR--an evidence-based program listed on SAMHSA's National Registry of Evidence-based Programs and Practices, is effective in increasing participation in substance use disorder aftercare. Total of 180 participants (30 per tx provider) will be I.D.'ed by tx providers to participate during pilot year. Project the number of participants 280 during 2nd year of project/380 during 3rd year. In addition to all data required by this RFA, participants' attendance rates for therapy/support group meetings will be tracked for 1 year after release from intensive inpatient/outpatient tx, their substance use/abstinence, and substance use-related problems for 1 year. Data will be used to assess impact of project on 3 key outcomes: participation in aftercare tx/support groups, substance abstinence/ significant decrease in substance use, and substance use-related problems. Data will be compared to baseline data from 6 substance abuse tx centers. Expect 60% participant retention rate at 3 months post tx, and 40% retention rate 6 months post tx. Expect 75% of program participants abstinent from illicit substances 3 months post tx, 60% abstinent 6 mo post tx, 50% abstinent 1 year post tx.
